Please request repeat Prescriptions at least
48 hours before they are needed if possible.
If you take a medicine regularly it may be
added to your repeat prescribing record and
will be listed on a tick sheet which is
attached to your repeat Prescription.
Other Prescriptions that are short term or
occasionally required treatments (acutes)
may also be requested or reissued without
seeing the doctor. Occasionally the doctor
may ask that you make a telephone
consultation to further discuss your
request.
How to request a prescription:
1. Detach the tear off portion of your last
Prescription and tick the boxes
corresponding to the medicines required.
Hand this in to reception.
If you have difficulty coming to the surgery
you can send a request in by post. If you
also enclose a Stamped Addressed Envelope we
can send your Prescription out to you by
return.
2. Telephone the surgery and request a
repeat Prescription. Please check exactly
which medicines and doses are required
before phoning. This is a less preferred method
as mistakes are more likely.

4. If you prefer, you can arrange for
your local chemist to uplift your script
after ordering it through the surgery.
